BY SIGNING THIS FORM YOU ARE WAIVING ALL RIGHTS TO COMPENSATION IN CASE OF INJURY. I understand that my participation in a karate class, tournament, seminar, or special event (Fire Walk) associated with American Freestyle Karate Black Belt Club and or American Karate Academies National Association is completely voluntary and at my own risk. I also understand that no assurance guaranteeing my safety is being made and I agree to hold American Freestyle Karate Black Belt Club and or American Karate Academies National Association, the sponsors, facilitators, organizers and property owners completely harmless of all liability if I sustain any injuries. We will try to start the fire walk on time, but this is a fire and it must be exactly right to begin which could cause a delay.
Remember the black belt test will include the (7) AKANA kata’s and material the board wishes to see from the students own material. Refer to the AKANA web site at www.a-k-a-n-a.com/AKANA%20Membership%20Form.htm. The Gathering application along with the fee's will need to be received prior to December 15, 2009. If you need more time to get funds together from students..just let me know as I want all AKANA Black Belts to be part of our First ever AKANA Warrior Fire Walk. Included in the $150.00 registration fee (A) Tomoe test, (B) Seminar, (C) Fire walk and (D) Certificates of Gathering participation. Not included in the $150.00 registration fee (A) Tomoe Pendant's (Pendant's can be ordered after the test. It will take 8-12 week for delivery of the gold Tomoe’s, the pewter and silver will take between 4 to 6 weeks for delivery. The price will depend on the cost of the metals at the time of the order. (B) Black Belt test (Refer to the AKANA web site for information).
